摘要
探讨二维斑点追踪显像技术在(2DSTE)甲状腺功能亢进(甲亢)患者左心房功能评价中的应用价值。选取2015年6月至2018年6月本院甲亢患者100例作为甲亢组, 同期选取体检中心健康人员100例作为健康组, 采用2DSTE检测左心房容量[左心房最大容积(LAVmax)、最小容积(LAVmin)、左心房收缩前容积(LAVp)]、功能左心房被动射血分数(LAPEF)以及左心房主动射血分数(LAAEF)应变参数心室收缩期左心房整体应变率(sRs)、心室舒张早期左心房整体应变率(sRe)和心室舒张晚期左心房整体应变率(sRa), 分析2DSTE对甲亢患者左心房功能的评价价值。甲亢组LAPEF、LAAEF明显低于健康组, 甲亢组LAVmax、LAVmin、LAVp明显高于健康组, 差异有统计学意义(P<0.05);甲亢组sRs明显低于健康组, 甲亢组sRe、sRa明显高于健康组, 差异有统计学意义(P<0.05);Pearson相关性分析结果显示, sRs与LAPEF、LAAEF呈正相关, sRe、sRa与LAPEF、LAAEF呈负相关(P<0.05)。2DSTE对甲亢患者左心房功能具有良好评价价值, 其sRs、sRe、sRa可作为评价患者左心房功能的重要指标, 值得临床作进一步推广。
Abstract
The application value of two-dimensional speckle-tracking echocardiography (2DSTE) in evaluating the left atrial function in patients with hyperthyroidism was explored. During the period from June 2015 to June 2018, a total of 200 subjects were selected for this study. Out of 200 subjects, 100 were the patients with hyperthyroidism in our hospital, and 100 were apparently healthy individuals from the physical examination center (ascertained as controls). Three left atrial index were measured by 2DSTE, including the left atrial volume (LAVmax), minimum volume (LAVmin), left atrial pre-systolic volume (LAVp), function (LAPEF) and left atrial active ejection fraction (LAAEF), strain [global strain rate (sRs), early diastolic strain rate (sRe) and late diastolic strain rate (sRa)], which demonstrated the value of 2DSTE in evaluating the left atrial function in patients with hyperthyroidism. Compared with the healthy group, the LAPEF, LAAEF were significantly decresed in the hyperthyroidism group, while the LAVmax, LAVmin and LAVp significantly incresed. The differences were statistically significant (P<0.05). The sRs of the hyperthyroidism group was obviously lower than that in the healthy group, while the sRe and sRa in the hyperthyroidism group were significantly higher than those of the healthy group. The differences were statistically significant (P<0.05). The Pearson correlation analysis showed that the sRs were positively correlated with LAPEF and LAAEF, the sRe, sRa were negatively correlated with LAPEF and LAAEF (P<0.05). In this study, the results demonstrated that 2DSTE is an excellent technology in evaluating the left atrial function in patients with hyperthyroidism, and sRs, sRe, sRa can be used as the important indexes in evaluating the left atrial function of patients, which is worth for further clinical promotion.
